Sculpt Club Membership expires one calendar year from the date of joining, which is activated by full payment of a body contouring package purchase of $8000 or more.

Members may purchase body contouring/hair removal packages up until their last day of membership, and redeem the package after the membership is no longer active. Member special pricing only applies to packages and not single treatments.

All physical vouchers expire the same day that the membership expires. All physical vouchers in the swag bag are unique and if lost there are no replacement options. Voucher MUST be presented the day of the service or product pick up, or member will be required to pay for the product or service.

Vouchers have NO CASH VALUE, and Preventous will not offer substitutions or rain checks, not even for medical reasons.

Membership is non-transferable and not shareable, however, physical vouchers from swag bag can be used by any person who presents the voucher.

If a Membership Body Contouring package is cancelled/refunded in part or in full, for any reason at all including medical reasons, Preventous will retain the value of any products, services, and discounts that have been used, and issue a refund on the remaining balance.

By opting into this membership, the member is opting to receive communications and promotions via both email and text messages. It is the member’s responsibility to ensure Preventous has an updated email address and that Preventous is on the safe sender list. Members are welcome to call, text, email, or use live chat during business hours to inquire about any member promotions that are active.

Special membership discounts and promotions are based on the regular list price of the treatment or service. Member discounts cannot be combined with other offers or promotions.
